PROBLEM TO BE SOLVED: To provide a form check apparatus storing a motion picture video image when a predetermined subject is included in the motion picture video image. SOLUTION: The form check apparatus includes: a camera unit 4 for shooting a subject; a subject determination section 10 for determining whether the predetermined subject is included in a predetermined region of a motion picture video image shot by the camera unit; a storage section for storing the motion picture video image after the subject determination section determines that the predetermined subject is included; and a reproducing unit 6 for reproducing the motion picture video image stored in the storage section.
