PROBLEM TO BE SOLVED: To provide a delivery slip from which a recipient can easily take out a statement of delivery even if the statement of delivery is placed in a pocket formed between the delivery slips stuck on an object to be stuck and delivered. SOLUTION: A pressure-sensitive adhesive layer is formed at the back face side of a baggage sticking part of a slip substrate, and release paper is stuck so as to be releasable at the back face side of the pressure-sensitive adhesive layer. The release paper has a storing region for a purpose of inserting the slip substrate and a release region for releasing from the pressure-sensitive adhesive layer between the back face side of the release paper and the surface of the object to be stuck when the delivery slip is stuck on the object stuck by the pressure-sensitive adhesive layer, and a lateral U-shaped notch is formed in a state of having an opening at its upper side in the boundary between the storing region and the release region. In addition, in the release paper, a notch for cutting off a region including a part of the opening side of the lateral U-shaped notch is formed, and the region in the notch is formed so as to be separable in an adhering state with the slip substrate when a part of the slip substrate is turned up.
