<P>PROBLEM TO BE SOLVED: To provide a questionnaire management system for increasing the collection efficiency of questionnaires by distributing sample merchandise for free based on a request from a user. <P>SOLUTION: The sample merchandise questionnaire management system is configured to, when a sample merchandise request is made from a user, provide the next sample merchandise as long as the number of times of request of sample merchandise is equal to or less than the prescribed number of times, and the answers of questionnaires are obtained with respect to the previous sample merchandise, and to display the questionnaire screen of the previous sample merchandise when there exists no answer, and to provide the next sample merchandise after the answers of questionnaires are obtained.
