<P>PROBLEM TO BE SOLVED: To provide a three-dimensional image processing apparatus providing an image by which an object of interest can be easily observed and an X-ray diagnostic apparatus. <P>SOLUTION: Data of a plurality of projected images which are photographed from different directions, regarding the object of interest is stored in an image data storage section 11. A feature point coordinates acquisition section 101 acquires the three-dimensional coordinates regarding the axis of the object of interest on the basis of the feature information and photographing directions regarding the axis of the plurality of objects of interest on at least two of the plurality of projected images stored in the image data storage section 11. A coordinates transformation section 102 sets the three-dimensional coordinates system, which has the axis of the object of interest as one axis, on the basis of the three-dimensional coordinates information.
