PROBLEM TO BE SOLVED: To widen a pasting area of a slip piece to a delivery article when a part of a release mount releasably pasted on the slip piece before the slip piece is pasted on the delivery article is pasted on the delivery article with the slip piece. SOLUTION: A delivery slip has the slip piece 110 in which a delivery confirmation slip 112 is separably partitioned and formed and an adhesive 130 is applied on a region except the delivery confirmation slip 112 on one face, and the release mount 120 releasably pasted on one face of the slip piece 110. The release mount 120 has a separating part 122 a part of a peripheral edge of which is arranged on a region opposing the delivery confirmation slip 112 by opposing the adhesive 130, and which is separably partitioned and formed by a separating line formed on the release mount 120. The separating line includes a perforation 126 which comprises a Y-shaped cut part consisting of a V-shaped part and an I-shaped part continuously formed in the releasing direction of the release mount 120 from a release starting part, starting the release starting part, where the release mount 120 is released from the slip piece 110, with the V-shaped part.
