An image is formed without losing a moving image included in a structured document. An MFP receives print data and moving images from a PC, associates the moving images with second moving image identification information and stores the moving images associated therewith, embeds the second moving image identification information of the moving images stored and positional information included in the print data in still images included in the print data, and forms the still images on paper during the image formation.  Furthermore, an MFP (100) reads the still images formed on the paper during the image reading (S71), extracts the second moving image identification information and the positional information from the read still images (S73, S75), obtains the moving images identified by the second moving image identification information extracted (S74), and generates a structured document in which the moving images are associated with the still images so that the moving images are displayed in the position identified by the positional information of the still images (S89).
