An applicator for inserting a surgical component during keyhole surgery, e.g. a hernia web, has a rotating holder (3) on a support shaft (2) to insert the rolled up component via axial movement followed by a rolling action to spread and position the component. The component is secured into a slit (6) in the holder and the axial position is defined by ratchet grips (9).
