PROBLEM TO BE SOLVED: To provide a simply configured image reader which is capable of avoiding the problem of a phenomenon which rarely occurs due to an influence of heat generation of a lamp, and to provide an image forming apparatus using the same. SOLUTION: A first carriage 105 includes an insulating member 120. The insulating member 120 is partially provided in an area including a center part in a longitudinal direction of a lamp 32. Provision of this insulating member 120 prevents a glass support member 101a from coming into direct contact with an outer peripheral part of the lamp 32 even when the glass support member 101a is deformed downward (for example, the center part is bent) by thermal expansion due to heat of the lamp 32. Consequently, a protection tube of the lamp 32 is not damaged, and leakage does not occur. It is sufficient when a material of the insulating member 120 has an insulating property and endures a temperature in the vicinity of the lamp 32. By this configuration, the image forming apparatus has high safety to be free from leakage from an outer surface electrode of the lamp 32 when being used continuously for a long period of time.
