PROBLEM TO BE SOLVED: To provide an in-wheel motor with a reduction gear capable of reducing the thrust force applied to a bearing, miniaturizing the bearing, reducing the backlash to reduce the gear rattling noise, and suppressing the noise in a cabin. SOLUTION: In the in-wheel motor 11 with the reduction gear, a stator 15, a rotor 16, and a reduction gear 23 connected to the rotor are arranged in a wheel 51 of a wheel 12, and the wheel 12 is driven via the reduction gear. The in-wheel motor has one output unit 27 and the other output unit 28 of a motor output shaft 17 connected to the rotor. The reduction gear a first helical gear 31 provided on one output unit, a second helical gear 33 engaged with the first helical gear, a third helical gear 32 provided on the other output unit while being twisted in the opposing direction to the first helical gear 31, a fourth helical gear 34 engaged with the third helical gear 32, an output shaft 24 fitted to the second helical gear 33 and the fourth helical gear 34, and a bearing 35 supporting the output shaft 24.
