PROBLEM TO BE SOLVED: To easily form a crown restoration work model and conveniently execute extraction/insertion of a work model and a division denture mold using a tray body board of the forming tray as a base and repeated reproduction of the tooth row. SOLUTION: This forming tray comprises the tray body board A and its bottom cover plate B which are integrally molded of synthetic resin into corresponding and semi-elliptic planar shapes, an interval between a bordering wall 12 and a partitioning wall 13 facing to each other in the tray body board A is defined into a bottomless penetration state as a hollow S having a horse-shoe planar shape and a prescribed depth D, and the hollow S is filled with molding gypsum paste P of the work model 14 under a covering state formed by extractably inserting and fitting the bottom cover plate B into the hollow S of the tray body board A.
