PROBLEM TO BE SOLVED: To provide a distribution and transportation slip with a payment slip, which is favorable in feeding properties in a printer and in processing of the payment vote with an optical character recognizer and causes no waste of resources. SOLUTION: In the distribution and transportation slip with the payment slip, in which one-part format distribution and transportation slip part with a tack part provided on its rear surface, a payment slip part and a commodity specifications part are planarily and continuously provided in the order named in a unit document, a spacing sheet for adjusting the thickness with that of the tack paper is provided on the rear surface of the commodity specifications part.
