PROBLEM TO BE SOLVED: To improve positioning accuracy of a control band in accordance with increase in recording density of a magnetic disk. SOLUTION: In members 14 formed at opposite corners of a pair of piezoelectric elements 12 via a head slider 11 and configured integrally with top face parts 14a and blocking parts 14b covering the top faces and the side faces of the diagonal parts, respectively, the top face part 14a is connected to a support member 13. In members 15 formed the other corners of the pair of piezoelectric elements 12 and configured integrally with top face parts 15a and blocking parts 15b, the top face parts 15a are extended, the pair of piezoelectric elements 12 is connected to the head slider 11, and the piezoelectric elements 12 with voltage applied thereto control the head slider 11.
