<P>PROBLEM TO BE SOLVED: To streamline the input of information on findings. <P>SOLUTION: A display part 14 displays finding input screens and finding-related information input screens. The finding input screens include a schema input screen 2000. The finding-related information input screens include an order information input screen 3000, a summary information screen 4000 and a practice schedule screen 5000. A storage part 12 stores correspondence information 120 associating finding entries and finding-related information entries.
