PROBLEM TO BE SOLVED: To provide a defect list of a 2ECC size or more, and an information recording medium capable of dealing with the delete of a defect entry in the defect list. SOLUTION: The information recording medium 100 is provided with a data area 102 for recoding user data, and at least one defect management area 104 (105, 108, 109) for recording a defect list 112 to manage N pieces of defect areas (N is an integer satisfying N≥0). The defect list includes a header 121 located at a fixed position, n pieces of defect entries 122 to 125 containing information regarding the positions of defect areas, and an anchor 126. The header, the N pieces of defect entries, and the anchor are arranged in this order. The header includes a defect list identifier 131 for identifying a defect list, a first update-times information representing the number of times that the defect list has been updated, and a defect entry number 132 representing the number of defect entries.
