<p><P>PROBLEM TO BE SOLVED: To provide a medical cooperation support system for quickly finding out a proper hospital transfer destination corresponding to the conditions of the patient, and for reducing the labor of the operation of a welfare undertaker such as a case worker when it is necessary to transfer a patient from an acute phase type hospital to a recuperation type hospital. <P>SOLUTION: This medical cooperation support system is provided with: a server equipped with a storage means for storing various pieces of facility information owned by a medical facility as a database; and a user terminal which can be connected through a network to the server.
