<P>PROBLEM TO BE SOLVED: To prevent a connection tube par, to which a supply side line and a patient side line are connected, from being damaged by an external force when an instrument for mixed injection is configured by integrating a plurality of mixed injection port configurating parts. <P>SOLUTION: Between supply side and patient side mixed injection port constituting parts 11 and 12, a flow path forming part 13 communicated with the inside of both mixed injection port constituting parts 11 and 12 is provided. The supply side mixed injection port constituting part 11 is provided with a supply side connection tube part 22 to which the supply side line is connected. The patient side mixed injection port constituting part 12 is provided with a patient side connection tube part 40 to which the patient side line is connected. A base part 10 for connecting the supply side and patient side mixed injection port constituting parts 11 and 12 is provided.
