A slide type fork device is provided to make the use easy and to prevent a dust from being raised. The lengths of a base side member(15), a middle associating member(17), and a tip end member(19) in the slide direction are the same. The lengths of a base side associating member(16) and a tip end side associating member(18) in the slide direction are the same and smaller than the middle associating member. The base side member, the base side associating member, and the middle associating member is associated with a base side rack, a base side relay rack, and a base side pinion. The base side associating member, the middle associating member, and the tip end associating member are associated with a chalaza. The tip end side associating member and the tip end member are associated with a tip end relay rack, a tip end side rack, and a tip end side pinion.
