PROBLEM TO BE SOLVED: To provide a hologram recording medium evaluation apparatus by which error generating possibility is eliminated and which has satisfactory evaluation accuracy and satisfactory reproduction accuracy in correspondence of each recording matter and each recording condition in multiple recording of a hologram recording medium, to provide a hologram recording medium recording and reproducing apparatus, to provide an evaluation method of the hologram recording medium and to provide a recording and reproducing method of the hologram recording medium. SOLUTION: The hologram recording medium evaluation apparatus is provided with a recording data forming means forming recording data from data prepared for recording and data about the recording condition to the hologram recording medium when data are recorded in the hologram recording medium and an evaluation means performing evaluation of the hologram recording medium based on the recording condition in the recording data reproduced from the hologram recording medium.
