A composition comprising the extract of Allium cepa L. skin for preventing and treating diabetes mellitus is provided to inhibit activity of alpha-glucosidase and rapid increase of glucose concentration in blood after meal without side effects, so that glucose concentration in blood is reduced. A pharmaceutical or health food composition for preventing and treating diabetes mellitus comprises 0.1-50 wt.% of the extract of Allium cepa L. skin with no toxicity and side effects, is prepared by extracting Allium cepa L. skin with water, C1-C4 lower alcohol including methanol, ethanol and butanol or a mixed solvent thereof, and has daily dosage of 0.0001-100 mg/kg, wherein the health food composition is formulated as powder, granule, tablet, capsule, syrup or beverage.
