<P>PROBLEM TO BE SOLVED: To provide an information storage medium having a forgery prevention function that can be easily added by transferring or sticking to not only a card but various high value-added commercial products. <P>SOLUTION: The information storage medium comprises: a substrate; a hologram region formed so that irregularity of diffraction-gratings regularly or irregularly provided on the surface of the substrate is observed as a hologram pattern by the reflected light from the irregularity of the diffraction-gratings; and a ferromagnetic thin film comprising an amorphous ferromagnetic material formed on the hologram region, wherein the ferromagnetic thin film holds characteristic magnetic properties according to the hologram pattern or the shape pattern of the hologram region as administrative information for forgery prevention.
