PROBLEM TO BE SOLVED: To provide an optical wiring board and an optical module that uses the same, capable of aligning an optical waveguide and an optical fiber at low cost. SOLUTION: A polymer waveguide 13, consisting of a plurality of cores 11 and a clad 12, is formed directed from the center on a support board 10 toward one side, and a plurality of convex positioning patterns 14 are formed from the center on the support board 10 to another side, by using the core material and the clad material that form the polymer waveguide 13. Optical fiber guide grooves 15 are formed between the positioning patterns 14, the optical fiber 20 is mounted in the optical fiber guide groove 15, and the optical fiber 20 is connected to each core 11 of the polymer waveguide 13.
