PROBLEM TO BE SOLVED: To form a protective film which has a prescribed and uniform thickness free of unevenness, highly accurately and simply, and to suppress a manufacturing cost, while improving productivity. SOLUTION: A hermetically sealed mold space with the prescribed thickness is formed between a transparent resin filling mount 33 and a substrate by elevating the disk substrate 37a having an information recording and reflecting film placed on a disk placing table 18. Mold is formed by filling UV curable resin liquid in a mold space by a UV curable resin filling nozzle 40 of a resin filling and peeling apparatus 30 provided to the resin filling mount 33, and the UV curable resin liquid is irradiated with a UV light by a UV lamp 54 and this UV curable resin liquid is cured and the protective film is formed.
