PROBLEM TO BE SOLVED: To provide a permanent form made of a fiber reinforced resin and used for reinforced concrete floor slabs, which form is light enough to be easily carried into construction sites, is permanently set and does not need to be removed after the placement of concrete, and moreover the form becomes part of concrete floor slabs, reinforces them and is free from corrosion or exfoliation from the concrete for a long time. SOLUTION: A plurality of square steel pipes 3 each covered with a fiber reinforced resin 2 and having outward protrusions 9 from both the upper ends thereof are placed in parallel at a fixed interval over a bottom plate 4 made of the sheet-like fiber reinforced resin 2.
