<P>PROBLEM TO BE SOLVED: To provide various convenient usages corresponding to desires in relation to forming of an image from a user without degrading productivity of an image forming apparatus. <P>SOLUTION: When image data of a confidential document is formed on a transfer paper sheet 5 having an IC chip mounted thereon which is supplied from a paper supply section 4 to be conveyed, information necessary for managing the confidential document is written to the IC chip by an information writing section 11 provided to a portion in front of a photosensitive body unit 6M. The information necessary for managing the confidential document is formed beforehand by an information forming section 10. The transfer paper sheet on which an image is formed passing through a writing unit 3, the photosensitive body unit 6, a transfer unit 7 and a fixing section 9, is discharged to a paper discharge section 2 through an information writing section 12.
