PROBLEM TO BE SOLVED: To provide an optical waveguide film which is very low-cost and capable of supplying electric power, and a module for optical transmission and reception which uses the same. SOLUTION: A polymer optical waveguide film 10 comprises a square type core part 18 extending along the length of the film, a square type power supply line 19 which extends along the length of the film and is arranged side by side with the waveguide core, and clad parts 16 and 20 which surround the core part 18 and power supply line 19. In the polymer optical waveguide film 10, a plurality of core parts 18 are arranged in parallel along the width of the film to form a plurality of optical waveguides in the film, and two power supply lines 19 are arranged across the plurality of core parts 18 in parallel along the width of the film.
