PROBLEM TO BE SOLVED: To provide an application form with a concealing section which does not take time and effort on its opening work while individual information is protected. SOLUTION: The application form 10 with the concealing section is equipped with a main body part 11, the concealing section 13 and so on. In the main body part 11, a plurality of paper pieces such as an application paper and a duplicate piece related to application are bound on its left end part, and respective paper pieces have stepwise different lengths in the width direction, and index parts 12 to be indexes are formed by the differences in their lengths. In the concealing section 13, top and ground parts of the upper side sheet base material 14 and the lower side sheet base material 15 are stuck together with a re-releasing paste 16, and the left end parts of both sheets are stuck on the left end part of the main body part 11 to form a bag-like part. The upper side sheet base material 14 is equipped with a printing region 17 for printing an address or the like, and a use method displaying column 18 for displaying how to use the application form. The bag-like part is equipped with a double-faced tape 19 for storing the main body part 11 inside and sealing the bag-like part, and a perforation mc for opening for opening the bag-like part.
