<P>PROBLEM TO BE SOLVED: To provide a disk-shaped recording medium having a rewritable layer to which index information can safely be rewritten without affecting the digital data of a recording layer. <P>SOLUTION: The disk-shaped recording medium 1, to which recording data are rewritably recorded, is provided with; a recording layer 3 to which the recording data are recorded; and a rewritable layer 4 which is pasted, via an adhesive layer 13, on the opposite side 2b to the face 2a on which the recording layer 3 is provided.
