Form rolling dies for worm shaft processing which can accurately manufacture a worm shaft by a form rolling method by allowing teeth formed on an admission part to reduce processing resistance, and can process worm shafts with high price competitiveness by mass-producing products with excellent and uniform dimensional accuracy are provided. In form rolling dies comprising an admission part(2) which has a predetermined inclined plane and is formed on the outer periphery of cylindrical form rolling dies(D) in an admission direction of a workpiece(4), a parallel part(1) formed on a central portion of the outer periphery of the form rolling dies, and a separation part(3) which has a predetermined inclined plane and is formed on an end of the outer periphery of the form rolling dies, wherein teeth(101,201) are spirally formed from an outer end of the admission part to an outer end of the separation part, form rolling dies for worm shaft processing are characterized in that: teeth(201) of the admission part are formed in such a shape that the teeth have deep valleys(202), high and sharp tips, and a gradually more inclined angle as it goes to an end of the admission part into which the workpiece advances; teeth(102) of the parallel part are formed in such a shape that the teeth(102) having shallow valleys(102) and low height are formed in a vertical state, and the valleys(102) and the teeth(101) are formed in a shape with gentle slope and wide width and connected to the separation part as it goes from the admission part to the parallel part; and the admission part and the separation part have the same length.
