<P>PROBLEM TO BE SOLVED: To provide a preferable image for obtaining the state of an apparatus by a user, and to provide its forming method. <P>SOLUTION: The image forming apparatus outputs a status sheet SS2 expressing the state of each part and each developing device of the apparatus at need. The image forming apparatus provides test patterns 410, 400 and 420 comprising two band-like patterns formed to use two developing devices corresponding to toner colors (magenta and yellow) in which a toner residual amount out of the four developing devices is a prescribed value or less (end state) on a center, a right end, and a left end of the status sheet SS2. The image forming apparatus displays information 453 for specifying the developing device used for formation of the pattern in the vicinity of the upper end of each band-like pattern.
