PROBLEM TO BE SOLVED: To injection mold a disk substrate in which the information arrays formed on a stamper are transferred in a small offset. SOLUTION: The fixed mold is provided with a first centering mechanism which rotatively moves the stamper holding member 13 holding the stamper S on the fixed mirror surface block 14 and a second centering mechanism which linearly moves the holding member 13 to the radial direction. The first centering mechanism is so constituted that the teeth, formed in the form of a ring along the outer peripheral face of the holding member 13, are meshed with the worm gear arranged in a proper place in the mold. The second centering mechanism linearly moves the holding member 13 to the radial direction by free slidably inserting the capsule-like protrusion member 17 into the through hole which is formed in the sleeve 12 to the radial direction, making a sprue bushing 11 whose outer peripheral face has the inclined face ascend to the sleeve and pressing the protrusion member 17 by the inclined face.
