The test booklet includes a cover leaf and test sheets joined with the cover leaf in a fixed manner. The cover leaf comprises of three parts linked by respective torn lines to facilitate separation of each part based on requirement. The booklet can be refilled with sheets. The bottom portion of the cover leaf can be separated from the cover leaf for evaluation. Labels can be impressed onto the cover leaf for identification.
