<p><P>PROBLEM TO BE SOLVED: To provide a method and a system for applying for the payment of medical expenses and an application form for the payment of medical expenses, that make very easy the processes of applying for and determining medical expenses, while remarkably reducing the rate of erroneous application processes. <P>SOLUTION: A local device prints predetermined data on the application form for the payment of medical expenses, and encodes the predetermined data into QR codes for printing on the application form.
