PROBLEM TO BE SOLVED: To provide a method for manufacturing concrete specimens using a test record card for quality certificate of concrete in which data required for quality management of concrete is written and which is resistant to both alkali and water and capable of preventing peeling and breakage, and to provide such a concrete specimen. SOLUTION: Prescribed data required for quality management of concrete is written in the surface of a card main body made of chemical synthetic paper. The card main body is resistant to both alkali and water. An adhesive to be pasted to the surface of the concrete specimen is applied to the back surface of the card main body. The test record card comprises the card main body and a transparent resin film covering the entry surface of the prescribed data. After the test record card is pasted an inner surface of a specimen form, freshly mixed concrete is made flow in.
