PROBLEM TO BE SOLVED: To provide an optical disk recording and reproducing apparatus capable of easily analyzing the cause for a fault even in case of occurrence of the fault in an optical disk or even in case of occurrence of the fault in firmware. SOLUTION: A control information recording processing means 221 causes the optical disk 1 prepared with 64 bytes of regions for recording the title name to record at least version information of firmware of a front end, version information of firmware of a back-end, serial number information on the system concerned, as the control information used for analyses in case of the occurrence of the fault, in the remaining region of the region where the title name of the maximum 30 characters can be displayed with alphabetic characters on a display device 20, in the regions of the 64 bytes.
