FIELD: medicine, plastic surgery. ^ SUBSTANCE: one should perform exfoliation of skin-fatty flap at opening cartilaginous department of nasal tip, external surface of lateral pedicle of large alar cartilage and supplementary ones, and, also, inferior-external edge of pear-shaped foramen at affected side. Then one should substitute cartilaginous defect of nasal ala with material as a porous plate out of titanium nickelide of about 0.15-0.20 mm thickness, 3-4 mm width imitating external contour of cartilaginous skeleton of nasal ala. Implant should be applied with support onto the edge of pear-shaped foramen in projection of distal edge of additional alar cartilages and between median pedicles of pre-divided large alar cartilages. Then comes fixation of substitutional material towards cartilaginous part of nasal ala followed by wound suturing in. The method enables to prevent relapse of functional insufficiency of external nasal valve, that is, prolapse of nasal ala during inhalation due to substituting cartilaginous defect with super-elastic implant which should be additionally fixed to cartilaginous defect. ^ EFFECT: higher efficiency of reconstruction. ^ 3 dwg, 1 ex |
