PROBLEM TO BE SOLVED: To provide a form, the information printed at the inspection of the quantity of gas, water service, electricity or the like used in which can be easily concealed in situ and, in addition, the information printed in which can be detected when being glanced furtively. SOLUTION: This form is so constituted as to have a square form body 1 with a frontal surface 1a as a printed surface, a tongue piece 7 formed by making an incision 6 in the form body 1, a folding line 9 for turning up so as to oppose the surfaces 1a to each other by pinching one end 10 of the form main body 10 between the tongue piece 7 and the form body 1 and, at the same time, bonding surfaces 11 and 12 respectively having self-adhering material thereon are so constituted as to give self-bonding properties to the self-adhering material constituting both the bonding surfaces 11 and 12 at the position corresponding to the rear surface of the tongue piece 7 and on the rear surface of the tongue piece 7 when the form body 1 is turned up about the folding line 9.
