PROBLEM TO BE SOLVED: To provide a document processing apparatus capable of supporting a job for jointly creating a document by a plurality of staffs. SOLUTION: The document processing apparatus stores information for demarcating regions on a document and destination information for particularizing an information transmission destination in cross-reference with each other, reads a document image formed on a prescribed medium and a description information with respect to the document image, demarcates at least part of regions on the document image relating to the read description image, acquires the destination information stored in a storage means in cross-reference with the demarcated region, and provides the acquired destination information to prescribed transmission processing as information for particularizing the transmission destination of the read document image and description image.
