PROBLEM TO BE SOLVED: To obtain highly reliable, accurate data by reducing centering tendency in a questionnaire survey using SD method.

SOLUTION: A questionnaire sheet that is formed either electronically or as an object has a question area 10 and an answer area 12. The question area 10 has therein questions whose answers should be subjectively expressed in degrees by a respondent. The answer area 12 has therein a plurality of marks nonlinearly to enable degrees to be expressed according to the positions of the marks. The plurality of nonlinearly arranged marks in the answer area 12 are arranged in a circle shape such that no end points are highlighted.
