PROBLEM TO BE SOLVED: To provide a form rolling die capable of improving a deflection precision at the time of form-rolling a work, thereby improving the form rolling precision of the work to be form-rolled, and to provide a form rolling apparatus provided with the form rolling die. SOLUTION: When bolts 27, 28 are tightened, the clamping forces of these bolts 27, 28 become forces which act so as to close split grooves 13c, 14c by being transmitted from the tapered surfaces of taper holes 11a, 12a of form rolling die main bodies 11, 12 to the tapered surfaces of the taper-shaped parts 13a, 14a of taper sleeves 13, 14 with a split groove. When the split grooves 13c, 14c are closed, clearances between main shafts 17, 18 and the taper sleeves 13, 14 with the split groove are eliminated. Clearances between the taper-shaped parts 13a, 14a of the taper sleeves 13, 14 with the split groove and the tapered surfaces of the taper holes 11a, 12a of the form rolling die main bodies 11, 12 are eliminated.
