PROBLEM TO BE SOLVED: To provide a form device for setting a form at a desired location in a simple manner. SOLUTION: The form device 1 is for use in forming a continuous footing equipped with a footing section, and includes rise section forms 10 arranged in a manner opposed to each other, for forming a rise section of the continuous footing, footing forms 20 erected in a manner opposed to each other, for forming the footing section, and form fixing members 30 for coupling the footing forms 20 to the rise section forms 10, respectively. Each form fixing member 30 has a rise-side joint portion 31 connected to the rise section form 10, a footing-side joint portion 32 for pinching the footing form 20, and a footing width determining section 33 for determining the depth of the footing section by connecting the rise-side joint portion 31 and the footing-side joint portion 32 together.
