<P>PROBLEM TO BE SOLVED: To accomplish a questionnaire collection system allowing easy input to a questionnaire in an eating and drinking place such as a restaurant to improve a collection rate, and allowing an easy change of question contents according to order contents of a customer. <P>SOLUTION: This questionnaire collection system 1 comprises: an order slip issuing means adding order slip number information together with input information based on the order contents or the like of the customer, and issuing an order slip 9; a questionnaire information reception server means receiving questionnaire information inputted with the order slip number information added from a portable terminal 8 of the customer, and transmitting questionnaire information reception confirmation information corresponding to an order slip number; a questionnaire information reception confirmation means confirming that the questionnaire information corresponding to the order slip number is received from the portable terminal of the customer, on the basis of the questionnaire information reception confirmation information from the questionnaire information reception server means; and a checkout-counter billing and settlement means adding prescribed service and performing a billing and settlement process when the order slip confirmed with the reception of the questionnaire information by the questionnaire information reception confirmation means is paid for. <P>COPYRIGHT: (C)2006,JPO&NCIPI |
