<P>PROBLEM TO BE SOLVED: To provide an image forming apparatus which composes a test pattern original image for confirming a fault and a test pattern image for generating a plurality of part places, and outputs to record on one recording medium. <P>SOLUTION: A digital copying device 1 performs a predetermined image process on image data of a read original of an image reader 8 by an image processor 41 and outputs to record the image on recording sheet 21. When a fault confirmation mode is designated, the test pattern original image read by the image reader 8, the test pattern image for processing generated from an image memory 61 of the image processor 41 and a test pattern image for recording generated of an LED write control circuit 51 of an image forming unit 5 are superposed, compounded or separated and compounded, a composite image is generated. and the composite image is outputted to be recoded on one recording sheet.
