An implantable port (100) comprises a housing (101) having a proximal surfac e (114) which faces outward toward the skin and a distal surface which faces away from the skin. A side surface extends between the proximal and distal surfaces with a first well (102) formed within the housing including a first opening in the proximal surface. A second well (104) adjacent to the first well has a second opening in the proximal surface with first and second outl et openings (112) formed on the side surface of the port in fluid communication with the first and second wells, respectively, being separated from one another by a distance substantially equal to a distance separating lumens of a dual lumen catheter to which the port is to be connected.
