PROBLEM TO BE SOLVED: To provide a sheet for a concrete form capable of promptly corresponding to the alteration of a design like the scaling-up of a woody substrate or a change in outer dimension and having bending strength and excellent durability, its manufacturing method and the concrete form using the sheet. SOLUTION: An uncured molten sheet 6 comprising a polyolefinic resin 5 and a molded surface sheet 10 comprising a polyolefinic resin are laminated to be supplied to cooling rolls 13-15 while passed through the nip between a pair of squeeze rolls 8 and 9. Both sheets are pressed from the side of the molten sheet 6 by a molding roller 16 arranged in opposed relation to the cooling roll 13 positioned on this side of the curing region of the molten sheet 6 to form the groove part corresponding to the outer dimension of the woody substrate to the sheet in the longitudinal direction thereof.
