PROBLEM TO BE SOLVED: To provide a delivery form of a structure by which an agent copy slip is taken out as well in the stage wherein a delivery contractor pulls out a sales slip even if an agent forgets to take out the agent copy slip. SOLUTION: On a delivery form body comprising a pasting slip (11) and the delivery slip (13), at least the agent copy slip (31) which becomes the copy for the agent, and the sales slip (32) which becomes the copy for the delivery contractor are put in order, and one-end sections of respective slips are bound through one line of a first adhesive layer (41, 42, 43 and 44). Then, the delivery form is made tearable by a first perforation (71) for cutting off which is formed along the first adhesive layer (41, 42, 43 and 44). In the delivery form, a second perforation (72) for cutting off which is separate from the first perforation (71) for cutting off is provided on the agent copy slip. Then, a second adhesive layer (5) is provided between the first perforation (71) for cutting off and the second perforation (72) for cutting off to solve the above mentioned problem.
