[PROBLEMS] To provide a medicine research information collection form capable of effectively organizing the contents of the collected medicine research information collection forms into a database. [MEANS FOR SOLVING THE PROBLEMS] There is provided a medicine research information collection form (20) for medicine research requiring statistical processing for verifying the validity of the research result obtained from electronic input information generated according to handwritten information on the medicine research case. The medicine research information collection form (20) is a sheet of paper having a position encoding pattern P for deciding the coordinate position of an electronic pen (10) on the paper. Furthermore, the sheet of paper has a printed item for handwriting or selecting information on the medicine research such as clinical trial.
