PROBLEM TO BE SOLVED: To easily guide a leading end of a form deformed into a curled shape to a nip region of a fixing device even when such form is conveyed to the fixing device. SOLUTION: The fixing device X is provided with an image carrying side conveyance guide 60 for guiding a recording material to the nip region 52 of a fixing roller pair 50 on the unfixed image carrying side of the recording material between a prescribed transfer device and the fixing roller pair 50. As a result, even when the the paper curling to the unfixed image carrying side is conveyed to the fixing device, the image carrying side conveyance guide can appropriately guide the leading front portion of the form to the nip region 52 before the leading front portion of the form abuts on the heating rollers.
