<P>PROBLEM TO BE SOLVED: To provide creation of a versatile business form with a free layout by a user by using raw data directly connected with a work, and to provide easy saved location management of a created business form by providing tabulation conditions of the versatile business form and business form saved location information. <P>SOLUTION: A table selecting means 401 is provided for displaying a table list belonging to a database in a display device 10 on the basis of the constructed database 301, and selecting a table to be used. An extraction condition designating means 402 is provided for displaying items of the selected table and its attributes in the display device, and designating data extraction conditions. An editing condition designating means 403 is provided for designating output editing conditions of a layout and items of the business form. A condition saving means 404 is provided for saving the extraction conditions and the editing conditions in a storage. A business form output designating means 405 is provided for designating output by the series of operation.
