PROBLEM TO BE SOLVED: To provide an optical waveguide substrate which can securely be positioned with small optical axis deviation and has small transmission loss of light. SOLUTION: The optical waveguide substrate 10 is equipped with a base material 11, a film 31 where an optical waveguide is formed, optical elements 41 and 43, an optical path conversion part 22, a 1st engagement part 37, and a 2nd engagement part 22. The film 31 has a core 33 and a clad 34 and is joined with a main surface 12 of the base material 11. The optical elements 41 and 43 are mounted on the film 31 and optically connected to the optical waveguide. The optical path conversion part 22 converts an optical path where a light signal is propagated to a desired direction. The 1st engagement part 37 is provided on the film 31.
