<P>PROBLEM TO BE SOLVED: To provide an information card which can readily select information such as questionnaire or the like, with respect to cards having a choice such as a questionnaire sheet, an election sheet, an interview sheet or the like, and can read out/collect the selected information readily. <P>SOLUTION: The information card is provided with a choice part 10 where choices 10A, 10B, 10C are described, with choice lines a, b, c corresponding to the respective choices 10A, 10B, 10C in the choice part 10 and with chosen information accumulation parts 100A to 100G where the information finally attained by tracing the choice lines a, b, c and the selected information is accumulated.
